Dialysis Unit Physician Contact List Active Only - Alphabetical by Practice: Includes practice name, MD name, office line, staff Line, fax, pager, cell, and instructions. Drop down allows adding courtesy MD’s as needed

Note: The mailing label report is using 5160 label format. For printing out to the mailing label paper, select the report output to PDF format. When printing, select the "Page Scaling" to "None" in the "Page Handling" section of the Print dialog window.
